Did you know that 23.6 million children and adults in the United States, or 7.8% of the population, are living with diabetes? Alarmingly, 1/3 of those individuals are unaware that they have the disease! Putnam County Hospital recognizes the importance to support our communities with programs to help identify and support those with diabetes and to educate people regarding best health choices to assist in their prevention of diabetes.
The Diabetes Self-Management Education program of Putnam County Hospital is designed to provide the community with a resource for education and support to allow individuals and families in Greencastle, Cloverdale, Bainbridge, Reelsville and surrounding areas with diabetes to live to their greatest level of individual independence and wellness.
The DSME program includes team guidance from a Registered Nurse, Registered Dietitian and Rehabilitation Services staff. A physician order is required for program services which include, but are not limited to, self-management education, medication/insulin instruction, nutrition counseling, diabetes foot screening, continuous glucose monitoring and insulin pump support.
