- Exercise counseling and training: Exercise gets your heart pumping and your entire cardiovascular system working. You’ll learn how to get your body moving in ways that promote heart health.
- Education for heart-healthy living: Managing your risk factors, choosing good nutrition, smoking cessation…education about heart-healthy living is a key element of cardiac rehab.
- Counseling to reduce stress: Stress hurts your heart. This part of rehab helps you identify and tackle everyday sources of stress.
In addition to our comprehensive cardiac rehab program, our department offers extensive education to heart failure patients. Heart failure is a progressive medical condition, it requires a long-term commitment to care from both the patient and the health care team. Heart failure is one of the leading causes of hospitalization among older adults, and the number of people diagnosed with heart failure has increased significantly in recent years. Early recognition and treatment is critical to insure optimal quality of life for individuals diagnosed with heart failure.
Some symptoms of heart failure may include:
- Shortness of breath
- Fatigue, tiredness, loss of energy
- Loss of appetite, abdominal discomfort
- Abdominal bloating (stomach swelling)
- Swollen feet, ankles and legs
- Weight gain
- Cough
- Heart palpitations, fast or irregular pulse
Our trained nurses follow heart failure patients weekly after discharge for 30 days. Patients are taught how to live more comfortably with their disease, learn to recognize early signs and symptoms of heart failure and know when to seek treatment. Ignoring early symptoms and delaying treatment can lead to an emergency situation.
Putnam County heart failure services include:
- Patient and family education
- Weekly phone calls up to 30 days post discharge
- Weight and medication monitoring
- Activity/exercise instruction
- Referrals to support services if needed

Pulmonary rehab offers educational programs, monitored and supervised exercise and psychosocial support. Some of the educational programs include medications, understanding and using oxygen therapy, breathing retraining, strategies for managing breathing problems, diet and weight management. A physical activity plan is tailored to meet each individual’s needs.
