The Health Information Management (HIM) Department is here to assist in getting medical information that occurred during a visit to Putnam County Hospital. If you need assistance, come see us and we will help you in any way that we can. The Release of Information form can be filled out prior to visiting us to help speed up the process. Faxing the form is also available, please give us a call to confirm that it was received. Please allow us 24 business hours to complete your request. Also, please keep in mind that you will need a photo ID when getting records, this is to ensure regulatory guidelines for confidentiality are being followed properly.


Other forms available are the Complaint form, for notify the Privacy Officer of a HIPAA violation, and the form to Request to Amend Medical Record, for requesting information in the record to be changed. These forms can also be faxed or mailed into the HIM department.

PCH Patient Portal


In compliance with the federal government’s Meaningful Use Initiative, all hospitals will be required to electronically provide patients (and/or a patient-authorized user) access to their patient record. Having this electronic access through your personal, secured email will allow you, as the patient, the ability to review your medical record after your stay at Putnam County Hospital. This access provides you with information, including but not limited to, medical procedures completed, medical history, medications taken, allergies, existing or developing medical conditions, etc. You can also download your confidential medical record to your private computer for your personal records, as well as electronically share your record with another medical professional of your choice. Registering to access your medical records electronically will also give you the option to pay your Putnam County Hospital bill online. Putnam County Hospital is excited to offer these convenient services to our patients, and is here to help you through every step of the process.


Upon your discharge from Putnam County Hospital, the email address you shared with us during the registration process will receive an auto-generated email from our electronic health record. This email will direct you to step-by-step instructions on how to access your personal medical record.


You can also sign up for the Patient Portal by visiting Medical Records, located in the Ground Level (basement) of Putnam County Hospital. Face-to-face verification is required.
