Putnam County Hospital partners with Tx:Team to offer comprehensive inpatient and outpatient rehabilitation services. The hospital is dedicated to helping patients return to their home and regular lifestyle activities by maximizing their recovery potential.


Rehabilitation services provided are:

Patients now have DIRECT ACCESS to Physical Therapy. This means a physician’s referral is no longer required, and you are able to begin treatment right away to be one step closer to living pain free. Physical Therapist Assistant

Terry received his degree as a Physical Therapist Assistant from Vincennes University in Vincennes, Indiana. Terry has also been trained and completed education as Wound Care Certified, Certified Lymphedema Therapist, Certified Ergonomic Assessment Specialist, Certified Clinical Instructor, and a Certified Orthotic Fitter. He has a diverse background in patient care as evident in his experience working in a variety of settings including hospitals, nursing homes, and outpatient clinics. Terry is no stranger to the current and innovative treatment with complex open wounds. Having spent over 37 years treating wounds, he has learned the importance of using the most advanced equipment as Aqoustic Ultrasound Machine. Terry is very passionate about the treatment of wounds and its possibilities for healing. He strives to help his patients to achieve their goals of healing and his varied background provides the perfect foundation for helping to achieve those treatment goals.

Speech Language Pathologist


Occupational Therapist

Erin Winters, OTR/L received her Bachelor’s degree in Occupational Therapy from Indiana University in 1996, and prior to that had received her Associate degree in Physical Therapy from Vincennes University. Erin has experience in a variety of settings, including, outpatient, acute care inpatient, subacute care, skilled nursing, and home health care. This diverse background in settings and continuing education studies have enabled Erin to apply a variety of treatment techniques and modalities to orthopedic injuries of the shoulder, elbow, wrist and hand which includes custom splint fabrication.


Erin also enjoys helping patients with neurological diagnoses, conditions, or balance disorders reach their full potential for the highest quality of life that can be regained. She has been trained in Neuro-Developmental Treatment (NDT) and treatment of Visual Perceptual Impairment & Low Vision. Erin is also a Certified Mind, Body, Movement Specialist which integrates the application of Tai Chi movement into rehabilitation treatments for a wide variety of conditions.

Physical Therapist

Originally from Mumbai, Jaydeep received his Bachelor’s degree in PT in 2016 in India. In 2018, he graduated with a Master’s in Health Sciences degree from the University of Indianapolis and has been a practicing Physical Therapist since graduation.


Jaydeep has worked in a variety of settings ranging from orthopedic hospitals to skilled nursing. He specializes in treating orthopedic, neuromuscular conditions, sports injuries, joint replacements and any acute musculoskeletal conditions. He works with patients needing wound care for lymphedema, traumatic wounds, diabetic ulcers, venous stasis ulcers etc.
